Commercial Driver License Medical Examination

Medical Certification

 

Medical certification in accordance with Federal Motor Carrier Safety Administration (FMCSA) physical standards is required when the driver is operating a commercial vehicle in interstate commerce that:

  • has a combined gross vehicle weight or weight rating of 10,001 pounds or more

  • is designed or used to transport 9-15 passengers (including the driver) for compensation

  • is designed or used to transport 16 or more passengers (including the driver) whether for compensation or not

  • transports hazardous materials in quantities that require placarding under the hazardous materials regulations

Preparing for your visit:

You will need:

 

A Valid driver license, please bring with you to the visit

To complete the Driver information and health history sections of the Medical Examination Report prior to seeing the medical examiner

 

To bring a copy of your most recent Medical

examination report and/or CDL certificate

 

Copies of medical records from primary care providers that you have seen in the last 5 years, if available to bring the medication bottles for any prescription and or over the counter medications that you are currently taking or have taken within the last 30 days.

CDL Self Certifying

 

CDL Changes:

Self-certification: Every driver must certify the type of driving they do no later than January 30, 2014.

You must complete a self-certification application and submit it along with your medical card (if applicable). All incomplete applications will be rejected and may result in your driving privilege being cancelled.

All medical certificates issued on or after May 21, 2014, must be issued only by examiners listed in the National Registry of Certified Medical Examiners. See the DMV Bulletin and visit the National Registry for more information.
